My wifes Philippine passport is damaged, pages torn. We need to get a new one with her married surname from the embassy in London. We can't find this form online on the embassy's website or anywhere else. Although we can find it for embassy's in other countries. I emailed and they said its on their website, but it isn't. Does anyone have any advice? Thank you.
I'd take the wording from one of the other countries embassies and just redo it for the UK Embassey. Scanning the other country forms seems fairly generic wording. Could do it and drop them an email back with your form and note that form not on website and get confirmation that your form is acceptable
