That's pretty much the same form we completed a few years ago in Hong Kong and Timmers is right: "Foreign Service Post" does mean the name and place of the Philippine diplomatic mission accepting the completed application. Unusually for a Philippine government form, the date details are required in Day-Month-Year order rather than the more usual Month-Day-Year (aka American format).
I just wish they have guidance for answering the forms. Their website also needs updating. The list of requirements is different form the list on the form. Thanks for the clarification Markham
It has always been difficult to fill in Government forms, it takes a very smart person to compile a form that is easy to complete, that's why you'll never see any on the UKVI site

Hello Timmers and Anne, I was thinking, Timmers Wife apply her BRP when they are not yet married? Why your Maiden Name in your BRP? Am i right that you Apply the BRP after your marriage and with your Marriage contract so your BRP is Surname is your Husband??.. Im sorry im a bit confused now.... How come you need to change your Maiden Name in your BRP??
I will also need to Renew my Passport next year, it will be Expired Dec 2016, Since it take 4 months before i got my passport i need to apply a new passport by Sept.2016..
Hello again Sheryl. To make things clear, the Biometrics Residence Permit (BRP) will be given after you submitted your FLR application and it has been granted. You cannot have a granted FLR without getting married to your fiancé. I would like my married name to reflect in my BRP too. Unfortunately, the Home Office won't let me because my passport is in my maiden name. If I wish to change that, I got to renew my passport first.
Changing the name on a Philippine passport to my wifes married name at the Philippine Embassy in London - the e_Passport application is the only way to do it now as I understand it, what's not clear is what name to put on the form - I'm assuming it's the new (married) name but it's not clear (to me anyway!). Am I right?
We did ours at a Philippine Consular outreach here on the IOM. It can be done at any outreach. Its an alternative. Saved us a trip to London. I seem to recall we put the married name on the form. I also recall it was a poor form.
We will need to renew the Missus and nippers passports soon (next couple of months ) including a name change but of greater interest at what time does your old passport get returned as we need it for its link to the residence card and employment?
I seem to remember they both came back together, that is the old and new. The whole thing took 2 months from the point we made the application at the outreach, to receiving the passports. I actually phoned the Philippine Embassy and got some sense out them on this to topic.
Update on this - went to the embassy in London, we planned to go early and avoid the queues but to be honest neither of us could be bothered to get up that early! We arrived at 11am into Waterloo station and its a short ride to Charing Cross station on the tube - a lovely walk across Trafalgar Square and in a minute or two into the embassy. It was packed! After standing there for a minute or two wondering what to do someone pointed out we needed to take a ticket - the ticket machine is across the waiting room from the entrance so thats not immediately obvious - our number was 58 and the sign says currently serving no 18 - looks like its a long wait! It was actually quite quick, we got to the desk around 12:30 and after much filipino conversation (everyone talking all at once - how does that work?) the forms were accepted and the fee paid (in cash - HAS TO BE CASH!) we were told since it was almost lunchtime (1pm) to come back at 2pm and go upstairs for the photo but on the way out I took a chance and went straight up - luckily they let us in but turned away the next couple who came up so by 1pm we were all finished with a lovely sunny afternoon to do some sight seeing and meet up with friends. Not complicated at all, just got to wait now...
Yeah, for us it had to be cash and even though we used the Philippine Embassy Roadshow in the IOM they would not accept IOM pounds. So I had to toddle off to the nearest cash machine that gave out UK currency.
Thanks for the update and giving us the idea how it goes for passport renewal in the PH Embassy. If you don't mind me asking, how much is it now?
To update the passport its £43 CASH ONLY! But it cost us 3 times that just to spend the day in London had a good time though
