    Make sure you follow the guidelines for printing the affirmation. 1.5cm margin Times New Roman 12 point. You need to print the affirmation on ONE piece of paper but you can print on both sides. When I visited two weeks back there was a delay due to some emergency training. So, I was about an hour late being seen, even though I had the first appointment of the day.

    When you download the template it retains all required form attributes for editing / printing..
