From memory date of application - I seem to remember this is defined as date of payment and this is set in the immigrations rules as a definition and sets the dates for other documents and whether in time or not. If the TB certificate was valid when you applied and paid then probably ok. This is why key dates are when you paid the application fee and the date of the TB certificate.

Yes that is correct if the TB certificate was valid at application date no further action required
If not valid at application then attend biometrics and get a new certificate and upload
