Being dyslexic I find trying to work out and remember proceedings very difficult so I apologise for sometimes asking similar questions, I struggle to find the relevant information on line so I've called the ukvl helpline who I believe gave me the wrong information plus a call to ukvcas who helped a little, in my head it looks like this so far,
Fill in on line application and paying the NHS bill plus the visa bill plus we will be paying the extra £800 for the premium service, at the end of the application we will book our meeting and if lucky that will be free but depends on dates and time and can cost an extra £60-£120, we go to our appointment and hand over our paperwork and for an extra cost they will scan and upload? Or do we need to upload before we go there? After the appointment we will receive a message with the result of the application and then soon after the wife's BRP will be sent to us?
Please feel free to put me right on any of the above
Many thanks for your help and support

Once you booked an appointment you can upload before, but if you are paying for it, then take all your paperwork and they will only ask for what they need, I took loads of paperwork to be on the safe side, but they only uploaded what they needed,I did not pay for the premium and still got the visa before 3 weeks, the £60-£120 is for all the uploading and scanning the documents they do for you for your application.

Pay the application online at the same time you have to pay the ihs, then you pay for the premium and it will have dates you can choose to book, it will have additional extras like if you want them to scan and upload your documents, you take all your documents to the appointment once all this is done if your lucky like i was you will get an email within a few hours telling you what the decision was.Hope this is made it clearer for you and all the best.
